Background / Objectives

Metabolic organ cross-talk — coordinated dysfunction across hepatic, pancreatic, adipose and renal systems — precedes overt disease by months to years. Conventional laboratory thresholds detect isolated organ dysfunction but fail to capture convergent multi-system signals. We present BURAK, a fully deployed biocomputational software platform developed by Burakomics Sciences, a QDB Accelerator-selected deep tech precision medicine startup. BURAK ingests structured laboratory payloads and computes the Systemic Clinical Drift Index (SCDI): a deterministic, patient-specific measure of coordinated biological drift across metabolic cascade layers. The objective is to demonstrate that sub-threshold multi-organ cross-talk patterns are detectable through governed personal drift scoring without probabilistic inference.

Methods

The BURAK platform receives a structured patient payload — raw biomarker values, specimen timestamps and data quality metadata — and executes a 7-stage deterministic pipeline: source observation, reference comparison, direction adjustment, variability normalisation, underlying drift computation, bounded 0–100 transformation, and evidence state classification. Every output is arithmetically reproducible and traceable to its source. Three Sentinel domains assess cross-system concordance: Cardiometabolic (HbA1c, glucose, HOMA-IR), Hepatic-Lipid (LDL, triglycerides, ALT), and Renal-Metabolic (eGFR, uric acid, CRP). A Handshake layer quantifies inter-sentinel convergence. Outputs were evaluated against five controlled synthetic patient scenarios with pre-defined expected cascade states.

Results

Across all five synthetic scenarios, BURAK correctly identified concordant multi-system drift at sub-threshold marker levels and distinguished it from isolated single-organ elevation. In three scenarios representing early metabolic derangement, simultaneous Cardiometabolic and Hepatic-Lipid Sentinel activation was detected before any individual marker crossed a conventional clinical threshold. The platform produced fully traceable SCDI outputs with validity state, directionality registry version, and complete aggregation path from biomarker to final score in each case.

Conclusion

BURAK demonstrates that deterministic personal drift scoring can surface coordinated metabolic organ cross-talk before conventional thresholds are breached. Its fully traceable, model-free architecture makes every output auditable and clinically defensible. The platform is ready for prospective validation against MASLD progression, T2DM onset and cardiovascular endpoints in real-world clinical populations, with active deployment planned across hospital partners in the Gulf and South Asia.
